Annual Training

Every year, providers participating in the CDA Nutrition Program must complete a MANDATORY Annual Training before July 31, 2020.

All in-person training have been CANCELLED– all locations.
You can complete your annual training online.


Annual Training Online

It is mandatory that you complete the online nutrition annual training and quiz by July 31st, 2020. CDA is required by the Department of Education to declare any provider who has not completed the annual training Seriously Deficient. Follow the steps below to review the online training materials and take your quiz. Don’t forget to submit your completion form!

1. Review Materials

We have included the materials discussed at the live training. After you have read through the training, you will be able to complete the Annual Training Quiz.

2. Take Quiz

The quiz is automatically graded on the computer. When you put in your name and provider number (the same one used in Minute Menu) and finish the quiz, a message is sent to CDA notifying us that you have completed it.

3. Submit Completion Form

Once you have completed the training and the quiz, you must print, sign and date, then submit to our office the Provider Annual Training Form. Be sure to keep a copy of your signed Training Form for your records. We will send you a Training Certificate to acknowledge your successful completion of the training once we have received your signed form.
